Messages, documents & your records
Secure messaging that beats phone tag, your document locker, lab results, visit summaries, and records release.
Welcome
This guide covers Messages, your secure line to your care team, and where your documents and your records live in the portal.
Messages
Messages is in the menu, right under To-Do. Write your question, press Send, and replies arrive right here, each one showing who on your care team wrote it.
Use Messages, not regular email, for anything about your care. It's the secure way to reach us. For anything urgent, call the office.
Documents & forms
Documents & forms, under Paperwork, has four sections. Documents holds what your care team shares with you, and 'Upload a document' is the secure way to send us a completed form.
A consent waiting for you is under Consents. Press 'Review & sign', read it, type your name, and sign.
Need paperwork completed, like FMLA or disability forms? Under Forms, press 'Request a form'. Once your provider has signed it, you can download it right here.
Your record
My record, under My care, holds your clinical notes, your visit summaries, your lab results, and your intake. Pick a section at the top.
When your provider orders lab work, it appears on your To-Do list, with any preparation and how to schedule with Quest Diagnostics.
Sharing your records
To share your records with another provider, open Records release, under Documents & forms, and give your permission there. Your answers are saved as you go.
And if you're ever in crisis, don't wait on a message. Call or text 988, any time, day or night.
